PAYMENT INFORMATION Payment is required in full within 24 hours of receiving your confirmation email. Cost of clinics is $30 plus $25 Sports Connection Players Card Fee (annual). Once your Registration Form has been received and processed, you will receive an Email. with instructions to log into our secured online system to process your CJV Payment. Or you can mail your payment to Carolina Juniors PO Box 470881, Charlotte NC 28247. Parent's must have an Email address on file to access the online program.
CONSENT FORM All participants must have a Signed Release and Waiver Of Liability and Indemnity Agreement and Consent Form on file with Sports Connection before they will be allowed to participate in any Leagues or Other Activities associated with the Sports Connection, LLC. Participants under 18 years of age must have a parent or guardian sign the Sports Connection, LLC Release and Waiver Of Liability and Indemnity Agreement. I, (self/parent/guardian), desire for my (self/son/daughter) to participate in one of Sports Connection, LLC's or Charlotte Sports Center, LLC's leagues or other activities and have read and agree to the terms and conditions listed below regarding the Release and Waiver of Liability and Indemnity Agreement. I understand that playing in a sports league or other activity involves physical contact between players and I assume the risks associated with (my/his/her) participation. (I/My child) (have/has) no known medical conditions which prohibit participation. I agree to hold harmless and release Sports Connection, LLC, Charlotte Sports Center, LLC and their agents and sponsors from legal liability resulting from any injuries sustained during participation in these activities. I understand Sports Connection, LLC and Charlotte Sports Center, LLC cannot be responsible for loss or damage to (my/my child's) personal possessions.
